OFFICE POLICY

  • I make every effort to respond to requests for a call back within 24 hours.  If my expertise does not correspond to your needs, I may recommend a more appropriate referral to another provider.
  • Please give 24 hours notice for cancellations or you will be responsible for the full fee if appointments are broken (except in the case of an emergency).
  • Payments and copays are expected at the time of the session, unless other arrangements have been made in advance. Some insurance plans are accepted.  Please contact me to discuss insurance as participation in insurance networks changes from time to time.
  • If you need pre-authorization for services, please contact your insurance company prior to your first appointment
